This following page will cover information regarding the COVID-19 vaccinations for residents of Nagareyama.
Please understand that the following information may change from now on in relation to information from the national government and other factors.

About COVID-19 Vaccination Booster Shots (3rd Dose)

Target group
Those who are 18 years or older and who have already received their 2nd vaccination at least eight months ago are the target group as the general rule.
It is possible to receive the vaccination booster eight months after the same day you've received your second vaccination. It is also possible to get the vaccination booster on the 1st of the month following eight months after the day you've received your second vaccination if this is not the same day.

The Japanese government decided to shorten the interval of vaccination in order to take all possible measures against COVID-19 infection. Healthcare workers and nursing home residents who are at high risk of severe illness will now be able to receive booster shots 6 months after the date of the 2nd dose, and from February 2022, other elderly people will be able to receive booster shots 7 months after the date of the 2nd dose.

Vaccination booster type
The mRNA vaccine is used regardless of the type of vaccine given for your primary vaccination.
Currently, The Pfizer's vaccine and the Takeda/Moderna's vaccine  have received pharmaceutical approval as a COVID-19 booster dose in Japan.
(Futhermore, additional vaccines to be used are possible based on drug regulation approval and other factors from now on.)

Vaccination booster(3rd dose) ticket issuance timeframe

Tickets will be issued in order of those who have completed their 2nd vaccination.
Additionaly, there may be cases where tickets are not issued to your address if you have moved in Nagareyama City. Please contact the Nagareyama City health center if this happens.
If you do not receive your ticket after 8 months from the date you completed your primary vaccination, please contact the Nagareyama City health center.

Regarding those who received their 2nd vaccination from another government organization and then move to Nagareyama
A booster vaccination ticket will be issued based on your application. This tickets are planned to be issued to those who have already completed their 2nd vaccination in order of having had receives said 2nd vaccination. However, those who have moved to Nagareyama City or moved abroad recently must fill out an application for this ticket.

Booster(3rd dose) ticket application
A vaccination ticket is required for COVID-19 vaccinations. Please apply through one of the methods below if you need a ticket to be issued.
・Those who have moved to Nagareyama after receiving their 2nd dose in another municipality are also to have their vaccination ticket issued based on their application.
・Thank you for your understanding in advance as there are cases where a vaccination ticket will take a week or longer to be received.

Application methods
Please confirm which application method applied to your situation and then submit an application as there are different application methods and documents according to various situations.
COVID-19 vaccination booster(3rd dose) tickets are planned to be issued to those who have already completed their 2nd vaccination in order of having had received said 2nd vaccination. Before warned that some time might be required for your paperwork between the reception of your application and your ticket being issued.

Note: If you want to send it different address from the registered address on your resident card due to inevitable reason, please attach proof may be a receipt for urilities( gas/ electricity/ water) or mail that is addressed to the person who is applying for the vaccination ticket to be issued.

In the case where you have moved after having had received two doses from another governing body.

・Your vaccination ticket was lost, destroyed, or damaged
・You changed the address on your certificate of residency afer the ticket was issued
・You changed your name after the ticket was issued
・You went to get vaccinated, but only used the medical history section of the COVID-19 Vaccination Booster (3rd Dose) Ticket

Category 3


・It has been 8 months or more after your two doses, but you haven't received a ticket
・You received your two doses abroad
・You received your two dosed via a combination of vaccines abroad and in Japan
・You have received your two doses as part of a vaccination program aimed towards Japanese livinng abroad
・You are an USFJ employee and received your two doses through UDFJ
・You received your two doses as part of a clinical trial for a pharmaceutical company
Note: Acceptable vaccines are limited to Pfizer, Takeda/Moderna, and Astrazeneca.

2 Copy of proof of vaccination for 1st and 2nd doses such as a vaccination certificate or vaccination record form.
 

If the person to be vaccinated is applying


Send in a copy of your ID such as a copy of your foreign insurance card, my number card, or health insurance card.
Note: In the circumstance where you do not have a certificate of residency or are not part of a family registry and are applying for a vaccination ticket to be newly issued from Nagareyama City, attach proof may be a receipt for urilities (gas/ electricity/water) or mail that is addressed to the person who is applying for the vaccination ticket to be issued.

Send a copy of power of attorney along with a copy of representing party's ID such as a copy of their foreign residency card, my numbercard, or health innsurance cad.
Note: A certified document proving one's relation is required if a ticket is to be sent to the address of a guardian who is acting as a representative for an adult.

Vaccination consent

Getting Vaccinated is NOT mandatory. Alongside information regarding the COVID-19 vaccination being thoroughly provided, the vaccine will only be admistered to those who wish to receive it.
For those who wish receive the vaccine, alongside understanding both the benefits and risk of side-effects from being vaccinated against the virus, your personal consent is also required. Those who do not consent to being vaccinated will not receive the vaccine.

Vaccination process

1 Confirm when you can be vaccinated
Please confirm the order of your vaccination and wait until your turn comes.
Although it is possible that there will be a short waiting period, we will make sure that all those who wish to receive the vaccine will be able to receive it, in due course, so please be patient.

2  Make a reservation and get the vaccine
 Make an appointment directly with a medical institution by Internet, phone.
